Description
American Traveler is hiring an experienced RN Case Manager for an acute care hospital setting requiring 2+ years of recent case management experience and Epic proficiency.
Details
- Case Management department in an acute hospital setting
- Adult and geriatric patient population with a 1:25-30 case ratio
- Day shift schedule, 5x8 hours, Monday-Friday 8:30am-5pm, plus one full weekend every 4 weeks
- No call or holiday requirements
- Responsible for initial patient assessments and follow-up visits at minimum every third day
Requirements
- Active FL or Compact RN license required
- BLS certification via AHA required
- Minimum 2 years of recent acute care hospital case management and discharge planning experience within the past 24 months in a US hospital
- Experience in a large 300+ bed facility preferred
- Level 1 Trauma experience preferred
- Proficiency required in Epic, CarePort, AllScripts, PowerChart, and Microsoft Office Suite
- Degree from an accredited school required, or from an institution approved by the Florida Board of Nursing combined with qualifying experience
- Candidates from Operation Nightingale schools not accepted
- Experience with 1:25 patient ratios required
Additional Information
- Responsibilities include care coordination, continued stay reviews, discharge planning, needs assessments, DME ordering, plan of care development, and workers' compensation case management
- Knowledge of CMS, DRG, HIPAA, OSHA, and Joint Commission/National Safety Goals regulatory standards required
- Smoke-free facility
- Requires successful completion of a Florida AHCA Level 2 background screening, including fingerprint-based state and federal criminal history checks and abuse registry review
- Local candidates accepted within a 60-mile radius at a reduced rate compared to travel positions
- Former Broward Health employees must be separated from the facility for at least 1 year before returning as a traveler
