Description
American Traveler is hiring an RN Case Manager for an inpatient hospital setting requiring 3+ years of acute care case management experience and IL RN licensure.
Details
- Inpatient case management unit within a 205-bed hospital with a Level II NICU
- Serves an underserved adult and geriatric population, including complex cases such as unhoused and low-income patients
- May be assigned to PCU, MedSurg, or LDRP units; ICU assignment possible but unlikely
- Typical caseload of 14-20 patients
- Day shift schedule, Monday through Friday, 0800-1630, with occasional late days
- Weekend rotation required every 5th weekend, with Monday and Friday off following a worked weekend
- No on-call responsibilities
- May float to other inpatient units, though typically assigned to a specific unit
- Uses EPIC EMR and Careport for referrals
Requirements
- Active IL RN license required
- Minimum 3 years of acute care case management experience required
- ASN or higher accepted
- No specific certifications required
- Strong assessment, critical thinking, and interpersonal skills required
- Comfortable navigating insurance plans and collaborating with multidisciplinary teams and physicians
- Must be a strong team player
- Candidate's middle name/initial, DOB, and last 4 of SSN required for consideration
- Minimum of 1 professional reference dated within the last year, from a Supervisor, Manager, Director, or Charge RN
Additional Information
- Collaborate with hospitalists, other case managers, a case management assistant, and department leadership
- Assist with facility initiatives and help patients access needed resources
- Orientation includes NEO plus 3 department shifts, with 1-2 additional shifts possible if needed
- Training provided on facility-specific programs and resources
- Holiday coverage required for approximately 50% or 2 holidays during the contract
- Eggplant-colored scrubs or business casual attire required, along with a white lab coat
- Free parking available on site
- First-time travelers are welcome to apply
- Local candidates accepted at the same pay rate with no radius restrictions
- Former Advocate Health employees must have been separated for at least 12 months to be eligible
