Description
American Traveler is hiring an RN Case Manager for an acute care hospital assignment requiring 2 years of acute medical Care Coordination/Discharge Planning experience and prior travel experience.
Details
- Acute Care Facility setting within the Case Management/Utilization Review department
- Day shift schedule, 5x8 hours (8:00 AM-4:30 PM)
- Patient ratio of 1:25
- No floating expected to other units, though floating between sister facilities may be required
- Typically one full weekend required every 4 weeks based on department need
- Epic EMR experience required
Requirements
- Active CA RN license required, pending license accepted if licensed by start date
- Minimum 2 years of acute medical Care Manager experience performing Care Coordination/Discharge Planning
- Prior travel nursing experience required, though first-time travelers may be considered with screening
- Epic charting system experience required
- 2 professional references covering 1 year of employment within the last 3 years, may include Charge RNs/Leads
- Full SSN and DOB required for consideration
Additional Information
- Responsibilities include admission criteria review, care coordination, discharge planning, disease management, needs assessment, DME ordering, and plan of care development
- Familiarity with CMS, CPT coding, DRG, and HIPAA regulatory guidelines
- Business attire or scrubs acceptable dress code
- Must commit to working 3 of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
- RTO limited to no more than 7 days and must be requested upfront during submission
- Local candidates accepted at a lower bill rate within a 50 mile radius
- Former permanent staff must wait at least 3 months before returning as a traveler and must reside outside a 50 mile radius from the facility
- Candidates must disclose any prior work history at a Providence facility as permanent staff, traveler, or registry/per-diem
