Description
American Traveler is hiring a Registered Nurse - Case Manager for an acute care hospital role requiring 2 years of acute medical Care Manager experience in care coordination and discharge planning.
Details
- Acute Care Facility setting within the Case Management/Utilization Review department
- Day shift schedule, 5x8 hours from 8:00 AM to 4:30 PM
- Patient ratio of 1:25
- No floating expected within the primary facility, though floating between sister facilities may be required
- Typically one full weekend of work required every 4 weeks, subject to department need
- Uses Epic charting system
Requirements
- Active CA RN license required, pending license accepted if licensed by start date
- Minimum 2 years of acute medical Care Manager experience performing care coordination and discharge planning
- Prior travel nursing experience required, though first-time travelers may be considered with screening
- Experience with admission criteria, medical necessity determination per evidence-based guidelines, disease management, needs assessment, and DME ordering
- Familiarity with CMS, CPT coding, DRG, and HIPAA regulatory guidelines
- Experience in ICU, Med-Surg, and SDU/PCU/IMC/Obs specialty areas within an acute hospital setting
- 2 supervisory references covering 1 year of employment within the last 3 years in a comparable specialty/setting
- Full SSN and DOB required for consideration
Additional Information
- Responsibilities include care coordination, discharge planning, disease management, needs assessment, DME ordering, and plan of care development
- Business attire or scrubs accepted as dress code
- Must commit to working 3 of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
- RTO requests limited to no more than 7 days and must be submitted upfront
- Candidates must respond to a question regarding any prior employment history with Providence facilities as permanent staff, travel contractor, or registry/per-diem
- Local candidates accepted at a lower bill rate; radius rule of 50 miles applies
- Former permanent staff must wait at least 3 months before returning as a traveler and cannot reside within 50 miles of the facility
- First-time travelers welcome to apply with proper screening
