Description
American Traveler is hiring a Home Health RN Float for a 13-week assignment requiring at least 1 year of certified home health and case management experience along with an active MA RN license.
Details
- Home health setting covering the North Region including Beverly, Marblehead, Salem, and Swampscott
- Float role requiring travel to patient homes as assigned
- Day shift schedule from 8:00 AM to 4:30 PM
- Weekend rotation required once every 6 weeks
- Uses Epic for charting
Requirements
- Active MA RN license required (no conditional or pending licenses accepted)
- Both NURSYS and Board of Nursing verification required for licensure
- AHA-issued certifications only
- Minimum 1 year of experience specifically within a certified home health agency required
- Minimum 1 year of case management experience required
- Hospice, private care, or standalone case management experience does not meet qualification requirements
- Flu vaccine required with no medical or religious exemptions
Additional Information
- Assess patient conditions, implement care plans, and provide ongoing education and support to patients and families
- Perform admission processes and case management responsibilities per organizational policy and standards
- Maintain thorough and timely electronic documentation and interdisciplinary communication
- Local candidates must reside within 60 miles of Massachusetts General Hospital in Boston
- Start date contingent on credentialing clearance, with rescheduling possible if not cleared by the posted start date
