Description

American Traveler is hiring an RN Care Manager for an acute care case management position requiring 2 years of RN experience and NC or compact licensure.

Details

• Acute care hospital setting within a 20-25 bed Care Management unit
• Covers Cardiology, Nephrology, Trauma Surgery, Ortho Surgery, Vascular Surgery, Neurosurgery, GI Surgery, Urology, Transplant, and ICU/Stepdown services
• Patient population is acutely ill with varying complex medical needs
• Uses Epic EMR for documentation and assessment
• Schedule is Monday through Friday, 8:30 AM to 5:00 PM
• No weekends, holidays, or on-call responsibilities required

Requirements

• Active RN license in North Carolina (Standard or Compact)
• Minimum 2 years of RN healthcare experience
• Nursing Diploma, ADN, or ASN from an accredited nursing program required; BSN or MSN preferred
• Strong assessment and critical thinking skills required
• Epic EMR experience preferred

Additional Information

• Conduct comprehensive assessments and develop individualized care plans to support case management, utilization review, and discharge planning
• Participate in daily Care Management touchpoints and interdisciplinary CAPP meetings to review patient progression and discharge readiness
• Attend weekly Complex Care Meetings to collaborate on solutions for high-risk and complex cases
• Coordinate with Social Workers, Utilization Managers, and Care Management Assistants on patient needs and administrative tasks
• Educate patients and families on community resources and coordinate referrals to post-acute care providers
• Experience in academic or teaching hospital environments is helpful
• Strong communication and coordination with interdisciplinary care teams required
