Description
American Traveler is hiring an experienced RN Case Manager for an Acute Care facility, requiring 2 years of Acute Care Case Management/Discharge Planning experience and prior travel nursing experience.
Details
- Acute Care hospital setting within the Case Management/Utilization Review department
- Day shift schedule, 5x8 hours from 8:00 AM to 4:30 PM
- Patient ratios of 1:25 on weekdays and 1:45 on weekends
- Every other weekend work required to meet department needs
- No floating expected
- Epic EMR experience required
Requirements
- Active CA RN license required, pending license accepted if licensed by start date
- BLS certification required
- Minimum 2 years of Acute Care Case Management/Discharge Planning experience required
- Behavioral health experience does not meet requirement
- Hospital experience required
- Prior travel nursing experience required, first-time travelers not considered
- Two supervisory references covering 1 year of employment within the last 3 years required
- Must provide SSN and DOB for compliance
- Must disclose any prior work history with the healthcare system, including permanent, travel, or per-diem roles
Additional Information
- Responsibilities include care coordination, discharge planning, disease management, needs assessment, DME ordering, plan of care, and prior authorizations
- Familiarity with CMS, Department of Health, DRG, and Joint Commission/Core Measures regulatory standards
- Business attire or scrubs accepted as dress code
- Must commit to working 3 of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
- RTO limited to no more than 7 days, must be requested at time of submission
- Local candidates accepted at a lower bill rate
- Candidates must reside outside a 50-mile radius of the facility to qualify for travel rate
- Former permanent staff may return after a 3-month break, provided they are not within 50 miles of the facility
- Travelers may be required to float between nearby sister facilities within the health system
