Description
American Traveler is hiring an experienced RN Case Manager for a hospital care management position requiring 3+ years of acute care RN experience and a BSN.
Details
- Hospital Care Management unit within an acute care hospital setting
- Day shift, 5 days/week, 8:00a-4:30p (40 hrs/week)
- Uses Epic for charting
Requirements
- Active RN license in Montana or via compact required
- BSN required
- AHA certifications required (BLS/ACLS as applicable), must not expire prior to start
- Minimum 3 years of RN experience required, acute care setting required
- Hospital care management experience required
- 1 year of experience in the specialty being submitted for is required
- ACM or CCM certification preferred but not required
- Knowledge of medical terminology required
- Proficiency with desktop software including PowerPoint and Excel required for data entry
- DOB and last 4 of SSN required for consideration
Additional Information
- Coordinates care and services for patients identified as needing case management assistance
- Assesses patients' medical, functional, and social needs to develop individualized care plans
- Coordinates discharge planning and collaborates with physicians and multidisciplinary care team
- Identifies strategies to reduce length of stay and improve appropriate utilization of services
- Advocates for patient rights and monitors care outcomes through variance tracking and data collection
- Provides education to physicians and ancillary staff regarding case management and discharge planning processes
- Participates in multidisciplinary rounds, case conferences, and guideline development
- Permanent staff must be separated from any Intermountain facility for at least one year before being eligible for this travel assignment
