Description

American Traveler is hiring an experienced RN Case Manager for a Discharge Planning role in an acute care hospital, requiring an active MA RN license and case management experience.

Details

  • Case Management department in an acute care hospital setting, ideally a large teaching hospital with 500+ beds
  • Day shift, Monday through Friday, 40 hours per week (may be scheduled as 4x10s or 5x8s)
  • Uses Epic for charting

Requirements

  • Active MA RN license required (pending or conditional licenses not accepted)
  • AHA-issued certifications required
  • Minimum 2 years of case management experience required
  • At least 6 months of discharge planning experience in an acute care hospital setting required
  • Flu vaccination required with no religious or medical exemptions allowed

Additional Information

  • Responsible for discharge planning and case management duties within an acute care hospital environment
  • Local candidates within 60 miles of Boston may be considered at a reduced rate
