Description
American Traveler is hiring an experienced RN Case Manager for a Discharge Planning role in an acute care hospital, requiring an active MA RN license and case management experience.
Details
- Case Management department in an acute care hospital setting, ideally a large teaching hospital with 500+ beds
- Day shift, Monday through Friday, 40 hours per week (may be scheduled as 4x10s or 5x8s)
- Uses Epic for charting
Requirements
- Active MA RN license required (pending or conditional licenses not accepted)
- AHA-issued certifications required
- Minimum 2 years of case management experience required
- At least 6 months of discharge planning experience in an acute care hospital setting required
- Flu vaccination required with no religious or medical exemptions allowed
Additional Information
- Responsible for discharge planning and case management duties within an acute care hospital environment
- Local candidates within 60 miles of Boston may be considered at a reduced rate
- Time tracked via UKG/Kronos
