Description

American Traveler is hiring a Home Health RN Float for a 13-week assignment requiring active MA RN licensure and prior certified home care and case management experience.

Details

• Home health float position covering North Region towns including Boxford, Danvers, Hamilton, Lynnfield, Middleton, North Andover, North Reading, Peabody, Topsfield, and Wenham
• Day shift schedule from 08:00 to 16:30
• 13-week assignment length
• Weekend rotation required once every 6 weeks
• Ability to travel to patient homes as assigned
• Uses Epic for charting
• Uses UKG/Kronos for timekeeping

Requirements

• Active MA RN license required (no conditional or pending licenses accepted)
• Minimum 1 year of Certified Home Care experience required
• Minimum 1 year of Case Management experience required
• AHA-issued certifications only
• Flu vaccination required with no religious or medical exemptions allowed
• Primary source license verification required through NURSYS and BON within 30 days of start, updated upon expiration

Additional Information

• Assess patient conditions, implement individualized care plans, and provide ongoing education and support to patients and families
• Perform admission processes and case management responsibilities per organizational policies and standards of practice
• Maintain thorough and timely electronic documentation and interdisciplinary communication
• Deliver patient care consistent with community health practice standards
• Local candidates accepted with permanent residence within 60 miles of Massachusetts General Hospital in Boston, subject to a reduced rate
• Parking not validated at Boston locations; public transportation or paid public parking required
