Description

American Traveler is hiring an RN Care Coordinator for a Case Management/Utilization Review role requiring at least one year of recent acute care case management experience and a current CA RN license.

Details

• Acute care hospital setting within the Case Management department, with potential assignment to the Emergency Department
• Coordinates care and services for assigned patients throughout an episode of illness or treatment
• Caseload includes quality/utilization review, admission/discharge criteria, observation/1-day stays, and 30-day readmissions
• 8-hour shifts scheduled 8am - 4:30pm, including weekdays and weekends
• Uses Epic EMR

Requirements

• Active CA RN license required
• Current CPR certification required
• At least one year of recent Case Management experience in an acute care setting required

Additional Information

• Collaborates with patients, families, physicians, payors, and the multidisciplinary care team to coordinate services
• Advocates for patients and families throughout the episode of care until discharge or transfer to an appropriate level of care
• Initiates clinical pathways based on admission diagnosis and serves as a resource for utilization management
• No holiday coverage required
• Candidates must be W-2 employees of their staffing agency
