Description

American Traveler is hiring an RN Case Manager for a Utilization Review/Case Management role requiring NYS licensure, PRI certification, and 2 years of case management experience.

Details

Day shift schedule from 8:00 AM to 4:00 PM
Manages a caseload of approximately 14-18 patients
Supports safe and timely patient discharge transitions
Coordinates durable medical equipment (DME) and post-acute services
Identifies high-risk patients and discharge barriers
Collaborates with interdisciplinary care team members
Documents and coordinates care using Epic EMR
Preferred experience with Epic and CarePort transition tools

Requirements

Active NYS RN license required
NYS PRI Certification required
Screen Certification required
BLS certification required (AHA only)
Minimum 2 years of relevant experience required
Two professional references from previous managers required
Skills checklist required for consideration

Additional Information

Coordinates care planning and discharge logistics across an interdisciplinary team
Uses Kronos for timekeeping
Local candidates accepted within 60 miles at a reduced rate
Candidates with prior work history at Northwell or Nuvance Health locations are subject to an eligibility review process before consideration
