Description

American Traveler is hiring an experienced Case Management RN for a community hospital setting requiring CA RN license and prior case management experience.

Details

Case Manager unit in a community hospital setting
Adult and geriatric patient population
Day shift, 5x8 hours, 0800-1630
Patient ratio of 1:25 for both weekday and weekend shifts
Weekend coverage required on an alternating basis
No floating required

Requirements

Active CA RN license required
Current BLS certification
Minimum 2 years of experience as a Case Manager
Minimum 1 year of experience in an acute care setting
Community hospital experience required
Teaching hospital and Trauma Level I/II experience preferred
Previous charge experience preferred
Cerner charting system experience required
Skills in case management/utilization review, admission criteria, discharge planning, and plan of care

Additional Information

Responsibilities include case management, utilization review, discharge planning, and developing plans of care
