Description

American Traveler is seeking an experienced RN Case Manager for an acute care Care Coordinator role requiring prior Dignity Health/CommonSpirit experience and an active CA RN license.

Details

• Acute care hospital setting within the Care Coordinator/Case Management unit
• Uses Cerner EMR
• 8-hour day shifts scheduled 08:30-17:00
• Requires working 2 weekends per month
• 13-week assignment with 12 hours of orientation

Requirements

• Active CA RN license required
• BLS certification required
• LA City Fire & Safety certification required
• Minimum 2 years of RN experience required
• Solid experience in Care Coordination required
• Prior experience with CA Dignity Health or CommonSpirit facilities required
• Copy of driver's license required for consideration
• Two professional references required: one supervisor from within the last 12 months worked and one peer or supervisor from within the last 3 years worked, including employment dates and rehire eligibility

• Coordinate patient care and manage case load within an acute care hospital setting
• Candidates may not have been directly employed by any CommonSpirit-associated facility, including CHI or Dignity Health, within the past year
