Description

American Traveler is hiring an experienced RN Case Manager with discharge planning expertise and Epic charting experience for a day shift assignment.

Details

• Case Management unit in an acute hospital setting
• Adult and geriatric patient population
• Patient ratio of 1:25-30
• Day shift schedule, 5x8 hours from 8:30am to 4:30pm
• Every other weekend required as needed
• Hours may exceed 8 hours per day depending on patient load
• Case managers are typically assigned by geographic area

Requirements

• Active RN license (Compact or GA) currently in hand
• BLS certification required in hand at time of consideration
• Minimum 2 years of Case Management experience required
• Discharge planning experience required
• Epic charting system experience required (current or within the past 6 months)
• Prior travel experience required
• Candidates who have been remote for the past 6 months must have recent experience with Epic and Interqual within that timeframe
• Strong communication skills with providers, nursing staff, and patients/families required

Additional Information

• Responsibilities include care coordination, discharge planning, needs assessment, DME ordering, and plan of care management
• Familiarity with CMS regulatory guidelines preferred
• Previous permanent employees eligible to return as travelers after 1 year of separation
