Description
American Traveler is seeking an experienced RN Case Manager for an acute care hospital setting, requiring 2 years of acute care case management/discharge planning experience and a CA RN license.
Details
- Acute care hospital setting within the Case Management department
- Day shift, 5x8-hour shifts from 8:00 AM to 4:30 PM
- Patient ratios of 1:25 on weekdays and 1:45 on weekends
- Every other weekend required to meet department needs
- No floating expected within the facility, though floating between sister facilities nearby may be required
- Business attire or scrubs accepted
Requirements
- Active CA RN license required, pending license accepted if licensed by start date
- Minimum 2 years of acute care case management/discharge planning experience required
- Hospital experience required; behavioral health experience does not qualify
- Experience with Epic EMR required
- Must be an experienced traveler; first-time travelers will not be considered
- Must provide 2 supervisory references covering 1 year of employment within the last 3 years
- Must provide full SSN and DOB for compliance
Additional Information
- Responsibilities include admission criteria review, benefits eligibility, care coordination, discharge planning, disease management, needs assessment and DME ordering, plan of care development, prior authorizations, and regulatory compliance (CMS, Department of Health, DRG, Joint Commission/Core Measures/National Safety Goals)
- Clinical specialties supported include Emergency Department, ICU, Medical-Surgical, and Surgical Services
- Must commit to working 3 of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
- RTO limited to no more than 7 days; all time-off requests must be submitted upfront
- Must reside more than 50 miles from the facility to qualify for travel pay rate, though local candidates are accepted at a lower bill rate
- Former permanent staff may return as travelers after a 3-month waiting period, provided they reside more than 50 miles from the facility
- Must disclose any prior work history at a Providence facility as a permanent employee, traveler, or per-diem staff
