Description

American Traveler is hiring an RN Case Manager for a Home Health position requiring active Oregon RN licensure and prior acute care or home health experience assessing skilled care needs.

Details

• Home Health setting providing case management and utilization review services
• 4x10 day shifts with a weekend rotation approximately every 5 weeks
• No night shifts required
• Uses Epic EMR

Requirements

• Active Oregon RN license required
• BLS certification preferred
• CRASE training required (if assignment extends beyond 6 months)
• Minimum two years of clinical experience required
• Previous clinical experience in acute care or home health setting assessing skilled care needs and equipment/health maintenance resource needs required
• Case Management Certification preferred
• Previous discharge planning and/or case management experience preferred
• BSN/MSN preferred
• Bilingual English/Spanish preferred
• Ability to lift up to 50 lbs independently and assist with patient transfers as needed

Additional Information

• Assess patients' healthcare needs and develop comprehensive, interdisciplinary care plans in collaboration with patients and families
• Coordinate transitions of care between hospital, home, rehab, and long-term care settings
• Evaluate medical necessity for treatments and collaborate with insurance companies on authorizations
• Monitor length of stay and identify discharge barriers to ensure appropriate use of resources
• Educate patients and families on diagnoses, treatments, and self-care
• Connect patients with community resources and facilitate referrals to specialized services
• Maintain accurate documentation of assessments, interventions, and outcomes
• Participate in interdisciplinary care conferences and collaborate with physicians, therapists, and social workers
• Agency nurses may be floated to other qualified departments before core staff
• Ability to work with culturally diverse patient populations required
