Description
American Traveler is hiring an RN or Social Worker Case Manager for a long-term discharge planning role covering ER, ICU, and Med Surg units.
Details
- Case Management department providing discharge planning coverage for ER, ICU, and Med Surg units
- Does not include Utilization Management duties
- 1st shift schedule, 8:00am-4:30pm, 5x8s totaling 40 hrs/week
- Weekend coverage required every fourth weekend
- No on-call responsibilities
Requirements
- Active MA RN license required, or Social Worker background accepted
- 3+ years of hospital Case Manager experience preferred
- Case Management Certification preferred but not required
Additional Information
- Ensures quality patient-centered care through daily case review and recommendations on length of stay and quality of care
- Collaborates with the healthcare team, patients, and families to assess, facilitate, and coordinate care across the continuum
- Provides post-acute hospital care planning and discharge facilitation, involving patients and families in the process
- Monitors discharge plans for variance and revises as needed to align with medical management initiatives
- Provides financial and regulatory information to patients and families regarding reimbursement sources
- Follows state and federal guidelines regarding patient rights in post-acute planning
- Serves as a clinical resource to the healthcare team and continues developing clinical knowledge
- Collaborates with community case managers and healthcare advisors for safe care continuity
- Conducts assessments and implements interventions for patients at risk of readmission
- Actively participates in daily interdisciplinary unit progression rounds focusing on GMLOS
