Description
American Traveler is hiring an RN Inpatient Care Manager for an acute care case management role requiring prior hospital discharge planning experience and an active NJ or compact RN license.
Details
- Inpatient Care Management department within a hospital setting
- Coordinates care and services for hospitalized patients to ensure safe and timely transitions
- AM shift schedule, 8a-4p, 7.5 hours per day, 5 days per week
- On-call responsibilities required
- May float to sister hospital as needed
- Uses Epic EMR for charting
Requirements
- Active RN license (NJ or Compact)
- Graduate of an accredited school of nursing; BSN preferred
- Prior hospital experience with discharge planning required
- Prior clinical experience in the care and management of hospitalized patients required
- Acute care case management experience preferred
- Case management certification (CM) preferred but not required
- Case management training from a professional case management organization preferred
- Strong communication, negotiation, and conflict resolution skills
- Data and computer proficiency required
- Knowledge of relevant state and federal regulatory standards related to case management, including CMS, Medicare, and The Joint Commission guidelines
Additional Information
- Develops and implements comprehensive case management and transition of care plans in collaboration with the interdisciplinary healthcare team
- Identifies high-risk patients and refers them to social services as needed
- Implements strategies to prevent hospital readmissions
- Monitors patient length of stay and actively participates in care rounds to meet clinical targets
- Communicates with healthcare team and patients regarding financial or payer-related issues impacting post-acute care plans
- Provides patient and family education on care progression, utilization of services, and safe transitions
- Identifies and escalates potential risk management or quality issues through appropriate channels
