Description

American Traveler is hiring a Case Management RN for a day shift position with weekend rotation, requiring at least 1 year of recent case management experience and a compact or CO RN license.

Details

• Case Management department with units of approximately 20 beds each
• Caseload ratios of approximately 1:20 for RNs and 1:40 for social workers
• Day shift schedule, 8:00 AM to 4:30 PM, with weekend rotation
• Uses mobile devices and computers for charting
• EMR systems: Meditech and CarePort

Requirements

• Active RN license (CO or Compact) required
• Current BLS certification required
• Minimum 1 year of recent Case Management RN experience required
• Discharge planning experience preferred
• ED experience preferred
• Palliative care knowledge and goals of care conversation experience preferred
• US Social Security Number and date of birth required for consideration
• No employment gaps of 90 days or more within the past 12 months

Additional Information

• Conduct discharge planning assessments and coordinate discharge planning for patients
• Collaborate with a team of case managers and social workers to manage patient caseloads
• Adaptability to work across different units is valued
• Strong communication skills and a professional, friendly demeanor are preferred
• Candidates must reside more than 50 miles from the facility; no local candidates will be considered
