Description
American Traveler is seeking a Home Health Physical Therapist with at least 1 year of experience and HCHB charting system proficiency to serve a geriatric and adult patient population.
Details
- Home health setting covering a multi-county service area
- Day shift schedule, 5x8 hours, 8:00 AM-4:30 PM
- Patient ratio/census of 1:5
- Adult to geriatric patient population
- Mileage reimbursed at IRS standard rate, tracked through HCHB/EMR system
- No weekend shifts required
- Floating required within facility/scope of practice as needed
- Facility transitioning charting systems from Homecare Homebase (HCHB) to Epic around September 2026
Requirements
- Minimum 1 year of home health physical therapy experience required
- Current BLS certification required
- GA state license or compact PT license required (license required if applicable)
- Homecare Homebase (HCHB) charting experience highly preferred; if not already proficient, completion of an online HCHB course is required prior to starting
- Experience with orthopedic and neurologic patient populations preferred
- Experience with OASIS documentation preferred
- Skilled in home exercise program development and patient/family education
Additional Information
- Primary responsibilities include home-based evaluation, treatment, and documentation for adult to geriatric patients
- Scope includes orthopedic conditions (joint replacements, spinal conditions, fractures) and neurologic conditions (stroke, Parkinson's, TBI, spinal cord injury)
- Utilizes adaptive equipment and assistive devices including walkers, gait belts, slings, and lifts as needed
- Scrubs of any color accepted
- Maximum RTO of 7 days per contract, with RTO finalized at time of submission and no changes allowed post-offer
- No RTO permitted during orientation week
- Holiday coverage required based on number of major holidays falling within the assignment period
- First-time travelers are accepted and will be screened for a 6-month experience baseline
- Local candidates accepted at the same bill rate
- Returning staff or travelers must meet facility-specific return eligibility requirements
