Description
American Traveler is hiring an experienced RN Case Manager for a 13-week day shift assignment requiring a CA RN license and 2+ years of case management experience.
Details
- Acute hospital setting within the Case Management unit
- Day shift schedule, 5x8-hour shifts (8:30 AM - 5:00 PM)
- Patient ratio of 1:20
- Manages complex patients with chronic or complicated medical conditions
- Coordinates with an interdisciplinary team to develop comprehensive discharge plans
Requirements
- Active CA RN license required at time of submission
- BLS certification required
- ACM and Interqual certifications preferred
- Minimum 2 years of case management experience required
- Any charting system experience accepted
- Community hospital experience preferred
- Full date of birth and full SSN required for consideration
- Two supervisor references preferred
Additional Information
- Conducts initial patient assessments and ongoing stay reviews
- Evaluates patient needs and coordinates discharge planning for complex cases
- Works to ensure patients are discharged to the appropriate level of care
- Candidates must reside within a 75-mile radius of the facility
- RTO requests always require facility approval and must be submitted at time of consideration
- First-time travelers are not accepted for this role
