Description

American Traveler is hiring an RN Case Manager for a Home Health (VNA) position requiring an active MA RN license and prior home health experience.

Details

Home health (VNA Nursing) setting providing skilled care and case management to patients and families in their homes
Monday through Friday schedule, 8:00am-4:30pm, with a 1-in-4 weekend rotation requirement
Uses Epic for charting

Requirements

Active MA RN license required; conditional or pending licenses not accepted
Current BLS certification (AHA preferred; Red Cross accepted at most locations, though some locations require AHA only)
Primary source verification of license/certification required within 30 days of start, updated upon expiration (NURSYS and BON required)
Home health experience required
Flu vaccine required with no religious or medical exemptions allowed

Additional Information

Provide skilled nursing care and case management under the direction of the Clinical Team Manager, following State Nurse Practice Act standards
Deliver direct care, teaching, counseling, and rehabilitative measures to promote and maintain patient health
Communicate and document patient observations and assessments according to accepted nursing standards and processes
Local candidates must reside within 60 miles of Massachusetts General Hospital in Boston
Parking not validated at Boston locations; public transportation and paid public parking lots available
