Description
American Traveler is hiring an experienced RN Case Manager for a Care Coordinator role requiring acute care case management experience and Epic proficiency.
Details
- Hospital-based Care Coordinator unit
- Day shift, Monday-Friday, 8:00am-4:30pm, no weekend rotation
- Nurse to patient ratio of 1 care coordinator per 16-25 patients per day
- 13-week contract duration
- Uses Epic EMR for charting
Requirements
- Active and clear NY RN license required
- BLS certification required
- PRI and Screen certification preferred
- Minimum 12 months of acute care nursing experience required
- Minimum 12 months of acute care case management experience in a hospital setting required
- Epic EMR experience required
