- Patient Population: Adults (18+), Cardiac, Ortho, Neuro, Debility, General Inpatient Rehabilitation
- Unit Size: 36 beds

Qualifications for RN Case Manager
- Minimum 2 years of case management or social work experience in an inpatient healthcare setting
- Experience in acute care or rehabilitation hospital preferred
- Leadership or supervisory experience highly preferred
- Current Registered Nurse (RN) license or Social Work licensure in Oklahoma
- Bachelor’s or Master’s degree in Social Work or Nursing
- Certification in Case Management or Rehabilitation (CCM, ARN, ACM, RN-BC) preferred
- Basic computer proficiency
- Strong verbal and written communication skills in English
- Team-oriented and cooperative attitude
- Compliance with influenza vaccination policy (masking required if exempted)
RN Case Manager Responsibilities
- Lead case management for inpatient rehabilitation patients
- Coordinate discharge planning and utilization review
- Facilitate family conferences and interdisciplinary team meetings
- Manage patient needs and resources for safe discharge
- Collaborate with the clinical healthcare team to optimize patient outcomes
- Document and manage patient information using EHR systems
