Qualifications for RN Case Manager

• Minimum 2 years of case management or social work experience in an inpatient healthcare setting
• Experience in acute care or rehabilitation hospital preferred
• Leadership or supervisory experience highly preferred
• Current Registered Nurse (RN) license or Social Work licensure in Oklahoma
• Bachelor’s or Master’s degree in Social Work or Nursing
• Certification in Case Management or Rehabilitation (CCM, ARN, ACM, RN-BC) preferred
• Basic computer proficiency
• Strong verbal and written communication skills in English
• Team-oriented and cooperative attitude
• Compliance with influenza vaccination policy (masking required if exempted)

RN Case Manager Responsibilities

• Lead case management for inpatient rehabilitation patients
• Coordinate discharge planning and utilization review
• Facilitate family conferences and interdisciplinary team meetings
• Manage patient needs and resources for safe discharge
• Collaborate with the clinical healthcare team to optimize patient outcomes
• Document and manage patient information using EHR systems
