Case Management RN Job Details
- Location: Fort Myers, FL 33907-3619 (Southwest Florida nursing jobs)
- Assignment Length: 13 weeks (January 5, 2026 – April 4, 2026)
- Estimated Weekly Pay: $1,687 – $1,874
- Schedule: Monday – Friday, 8:00 AM – 4:30 PM
- Shifts per Week: 5
- Shift Duration: 8-hour days
- Hours per Week: 40
- Weekend Requirement: One weekend shift per month
- On-Site Position
- Floating: Required to float to sister healthcare campuses as needed
Registered Nurse Qualifications
- Minimum 2 years of recent Case Management RN experience required
- Active Registered Nurse (RN) License (must be verified on Nursys and included in your profile)
- BLS certification required
- Care Management certification (ACM or CCM) preferred
- Degree accreditation must be included in your profile
- Ability to float to other healthcare campuses as needed
- Permanent address must be at least 50 miles from the facility (travel nursing requirement)
- All demographic information (address, email, full DOB, full SSN, RN license number) must be provided at time of application
- Hard document deadline is the Monday prior to start date
Case Management RN Responsibilities
- Deliver comprehensive case management and clinical care coordination services to patients, ensuring continuity of care and optimal outcomes
- Collaborate with interdisciplinary healthcare teams to develop, implement, and evaluate individualized patient care plans
- Coordinate patient transitions across care settings and manage discharge planning
- Advocate for patients and families, connecting them with appropriate healthcare resources and support
- Maintain accurate documentation and comply with all regulatory and hospital standards
- Float to sister campuses as required, functioning as a core staff member and supporting a positive healthcare team culture
