Job Summary
- Provide home health nursing care including patient assessment, care plan implementation, and ongoing education/support for patients and families.
- Perform admission processes and case management duties following organizational policies and community health standards.
- Deliver patient care consistent with organizational philosophy and standards.
- Utilize thorough and timely electronic documentation and interdisciplinary communication.
- Ability to travel to assigned cases.
- Weekend rotations occur approximately once every 6 weeks.
Experience Requirements
- Minimum 1 year Certified Home Care experience.
- Minimum 1 year Case Management experience.
Contract Length
- Approximately 13 weeks.
Credential and Compliance Requirements
Required for Submission
- License/Certificate Verification: Primary source verification of professional license/certification (MA Conditional Licenses not accepted). Both NURSYS and Board of Nursing verification required.
- Resume: Include past 7 years of work experience with city and state details.
- References: Two recent references, at least one supervisor or above in the relevant skillset.
- Skills Checklist: Completion of required skills checklist.
- Candidate Submission Coversheet: Current version required.
Required for Onboarding (must be completed prior to or at start)
- MGB Badge Photo: Recent color photo (no selfies, no filters, plain background).
- MGB Motor Vehicle Record (MVR) Check: To be run within 45 days of start; required for all home care positions.
- Fit Test: N95 respirator fit test within 365 days of start, updated annually.
- Nurse Aide Registry Check: Within 30 days of start, required by MA Department of Public Health for all employees.
- MMR Vaccination or Immunity Proof: Documentation of measles, mumps, rubella vaccination or immunity.
- Influenza Vaccine: Required during flu season (Sept 15 - April 30), no exemptions allowed.
- TB Test: Documentation of TB skin test or blood test within 3 months of start, updated if >6 month break.
- Health Statement: Physician statement within 365 days of start, updated if >6 month break.
- Criminal Background Check: Within 45 days prior to start, including international searches if applicable.
- Massachusetts CORI Check: Criminal history check within 30 days prior to start.
- Respirator Medical Clearance Attestation: Required if fit test is required.
- Confirmation of Assignment/POI Form: Required for all initial assignments and extensions.
- Education Verification: Verified through background check company (not required for CNA positions).
- SSN Trace: Must be run with criminal check, covering last 7 years.
- National Sex Offender Check: Within 30 days of start, updated annually.
- Exclusion Checks: Search of OIG, GSA, MassHealth, and NH Medicaid exclusion lists within 30 days of start and annually.
Optional / Preferred
- Hepatitis B Vaccination or Immunity Proof (strongly preferred).
- Tdap Vaccine within 10 years (preferred).
- Positive PPD with Negative Chest X-Ray documentation if applicable.
- TB Symptom Survey if positive PPD (optional).
Additional Notes
- No religious exemptions for fit testing or influenza vaccine.
- Fit testing cannot be performed onsite; must be completed prior to start.
- Weekend rotations are infrequent (once every 6 weeks).
- Travel to patient homes required.
This position is ideal for experienced home health RNs with case management skills who are comfortable working independently in community settings and meeting comprehensive credentialing and compliance requirements.
