Job Opportunity: RN Home Health

Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Job Summary

As a Home Health RN, you will:

• Assess patient conditions
• Implement care plans
• Provide ongoing education and support to patients and their families
• Perform admission processes and case management responsibilities
• Deliver patient care in accordance with organizational philosophy, policy, and community health practice standards
• Utilize thorough and timely electronic documentation and interdisciplinary communications
• Travel to cases as assigned
• Participate in weekend rotations once every 6 weeks

Experience Requirements

• Minimum 1 year Certified Home Care experience
• Minimum 1 year Case Management experience

No prior travel experience is explicitly required.

Credentials & Compliance Requirements

• Professional nursing license verification (Primary Source Verification required; MA Conditional Licenses not accepted)
• Submission of a recent color photo (within past 6 months) meeting specific guidelines
• Motor Vehicle Record (MVR) check within 45 days of start
• Criminal background checks including Massachusetts CORI, National Sex Offender Check, and Exclusion Checks (OIG, GSA, Medicaid lists)
• Resume including past 7 years of work experience with city & state
• Two recent references (minimum one supervisor or above) in the skillset
• Skills checklist required for submission
• Confirmation of assignment form upon acceptance and for extensions

Health & Safety Requirements

• Fit Test for N95 respirator within 365 days of start, updated annually
• Respirator Medical Clearance Attestation (if fit test required)
• Documentation of immunizations or immunity for:

  • MMR (Measles, Mumps, Rubella)
  • Influenza vaccine during flu season (Sept 15 - April 30) with no exemptions
  • TB test (skin test or blood test) within 3 months of start, updated if >6 month break
  • Health statement signed by MD/DO/NP/PA within 365 days of start, updated if >6 month break
• Hepatitis B vaccine strongly preferred
• Tdap vaccine preferred within last 10 years
• Additional TB-related documentation if positive test results

Additional Notes

• Ability to travel to patient cases as assigned is required
• Weekend rotations occur once every 6 weeks

This position is ideal for experienced Home Health RNs with case management skills who are comfortable with travel and comprehensive patient care in a community health setting.
