Job Opportunity: RN Home Health

Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (LTA)

Job Summary

As a Home Health RN, you will:

• Assess patient conditions
• Implement care plans
• Provide ongoing education and support to patients and their families
• Perform admission processes and case management responsibilities
• Deliver patient care in accordance with organizational philosophy, policy, and community health practice standards
• Utilize thorough and timely electronic documentation and interdisciplinary communication
• Travel to assigned cases as needed
• Participate in weekend rotations approximately once every 6 weeks

Experience Requirements

• Minimum 1 year Certified Home Care experience
• Minimum 1 year Case Management experience

Credentials & Compliance Requirements

• Valid professional nursing license with primary source verification (MA Conditional Licenses not accepted)
• Submission of a recent professional photo (color, plain background, no filters or eyeglasses)
• Motor Vehicle Record (MVR) check within 45 days of start
• Fit test for N95 respirator within 365 days of start, updated annually
• Documentation of immunizations and health screenings including:

  • MMR (measles, mumps, rubella) vaccination or proof of immunity
  • Influenza vaccine during flu season (Sept 15 - April 30) with no exemptions
  • TB test (skin test or blood test) within 3 months of start, updated if >6 month break
  • Hepatitis B vaccination or immunity preferred
  • Tdap vaccine within 10 years preferred
• Physician health statement within 365 days of start, updated if >6 month break
• Criminal background checks including:

  • National and Massachusetts CORI checks within 30-45 days of start, updated if >6 month break
  • Social Security Number trace for last 7 years
  • Exclusion checks (OIG, GSA, MassHealth, NH Medicaid) within 30 days of start and annually
• Submission of:

  • Resume with past 7 years of work experience including city and state
  • Two recent references (minimum one supervisor or above)
  • Skills checklist
  • Candidate submission coversheet

Additional Notes

• Ability to travel to patient cases as assigned is required
• Weekend rotations occur approximately once every 6 weeks
