Title: Physical Therapist (PT) – Skilled Nursing Facility

Location: Los Angeles, CA

Duration: 13 weeks (potential extension up to 26 weeks)

Shift: Day shift – 5x8 hours (8:00 AM – 4:30 PM)

Job Responsibilities

• Provide physical therapy services to residents in a skilled nursing facility

• Evaluate patients and develop individualized treatment plans focused on mobility, strength, balance, and functional independence

• Treat patients with a variety of post-acute and chronic conditions common to the SNF setting

• Document evaluations, treatments, and progress notes accurately and timely

• Collaborate with interdisciplinary teams to support patient-centered care and discharge planning

• Float to nearby skilled nursing facilities as needed to meet coverage requirements

• May provide occasional weekend coverage based on facility needs

Required Skills & Qualifications

• Active California Physical Therapist license in hand

• Experience working in a skilled nursing facility preferred

• Strong clinical assessment and treatment planning skills

• Ability to manage a diverse geriatric and post-acute caseload

• Effective documentation and communication skills

• Ability to float between facilities when required

Preferred Skills & Experience

• 1 year of SNF experience

• New graduate Physical Therapists considered

• Flexibility for assignment extensions and varied patient populations
