Title: Physical Therapist (PT) – Home Health / Hospice
Location: Dublin, OH
Duration: 13 weeks
Shift: Day shift, 5x8 hours (8:00 AM – 5:00 PM)

Job Responsibilities

• Provide physical therapy services to geriatric patients in a home health/hospice setting

• Manage an average caseload of 5–7 patients per day

• Complete Start of Care (SOC) visits when required

• Perform timely, accurate clinical documentation and maintain clear communication

• Collaborate with interdisciplinary care teams

• Participate in on-call rotation (approximately every 6th weekend)

• Travel within an assigned territory up to a 60-mile radius

• Adhere to professional standards, policies, and dress code requirements

Required Skills, Certifications & Qualifications

• Minimum 1 year of recent Physical Therapy experience

• Home Health and/or Hospice experience required

• OASIS experience required for SOC visits (Home Health)

• Proficiency in HCHB (Homecare Homebase) EMR required

• BLS certification required

• Active PT license in hand at time of submission

• Valid driver's license and reliable personal transportation

• Strong time management, documentation, and communication skills

• Ability to work independently in the field

• Willingness to be flexible with territory coverage

• NO first-time travelers
