Title: Occupational Therapist (OT) – Home Health / Hospice
Location: Dublin, OH
Duration: 13 weeks
Shift: Day shift, 5x8 hours (8:00 AM – 5:00 PM)

Job Summary

Seeking an experienced Occupational Therapist for Home Health/Hospice care. The role involves managing a caseload of geriatric patients within a defined territory and providing high-quality, timely therapy services. Candidates must be comfortable working independently, traveling within the assigned area, and maintaining accurate documentation.

Job Responsibilities

• Provide occupational therapy services to geriatric patients in a home health/hospice setting

• Manage an average caseload of 5–7 patients per day

• Complete Start of Care (SOC) documentation when required

• Ensure timely, accurate documentation and clear communication

• Collaborate with interdisciplinary care teams

• Participate in on-call rotation (approximately every 6th weekend)

• Travel within a territory up to a 60-mile radius

• Comply with dress code and professional standards

Required Skills & Qualifications

• Minimum 1 year of recent OT experience

• Home Health and/or Hospice experience required

• OASIS experience required for SOC visits

• Proficiency with HCHB (Homecare Homebase) EMR required

• BLS certification required

• Active OT license in hand at submission

• Valid driver's license and reliable personal transportation

• Ability to be flexible with territory assignments

• Strong documentation and communication skills

• NO first-time travelers
