Title: Physical Therapist (PT) – Skilled Nursing Facility (SNF)
Location: Lancaster, CA
Duration: 13 weeks (26 weeks preferred)
Shift: Day shift | 5x8-hour shifts (08:00 – 16:30)
May include occasional weekend coverage as needed

Job Responsibilities and Required and Preferred Skills & Requirements:

Responsibilities:

• Provide physical therapy services to patients in a skilled nursing facility setting

• Evaluate patients and develop individualized treatment plans to improve mobility, strength, and functional independence

• Deliver therapeutic interventions and monitor patient progress

• Document evaluations, treatments, and outcomes in accordance with facility and regulatory standards

• Collaborate with interdisciplinary healthcare teams to support patient care goals

Required Qualifications:

• Ability to work day shifts with potential weekend coverage

• Willingness to work in an SNF/LTC/LTAC care environment

Preferred Qualifications:

• Minimum 1 year of experience in LTC and/or LTAC specialty

• New graduates will be considered

• Flexibility with assignment length and scheduling
