Position: RN Case Manager
Facility: Upper Chesapeake Medical Center Bel Air
Location: 500 Upper Chesapeake Dr Bel Air, MD 21014
Shift: Day 5x8-Hour (08:00 - 16:30)
Duration: 13 Weeks
Start date: 02/23/2026

Position: Case Manager RN – Days
Hours: 40 hours/week | No weekends
Requirements:

Minimum 3 years of experience
First-time travelers accepted with inpatient hospital/acute care experience
EPIC charting experience preferred

Contract Details:

Start Date: 02/23/2026
Duration: 13 weeks
Schedule: 5x8 Day shifts, Monday–Friday block
- Shift hours: 08:00 or 08:30–16:00/16:30 (based on unit needs)

Licensure & Certifications:

Active MD RN license required
BLS preferred

Workload:

Average daily census: ~15 cases

Patient Population / Role:

Inpatient case management
Coordination of medical home care, placement, and social issues
Wrap-around services to support safe discharges
