Job Title: RN Case Manager (Inpatient)
Location: 500 Upper Chesapeake Dr, Bel Air, MD 21014
Duration: 13 Weeks | Start: 05/04/2026 or ASAP
Shift Details:

Day Shift: Monday–Friday
08:00 AM – 04:30 PM

Job Summary:
Seeking an experienced RN Case Manager (or SW preferred) with inpatient case management experience to handle complex discharge planning, legal coordination, and patient support services.

Responsibilities:

Manage ~15 inpatient cases daily
Coordinate safe discharge planning, placement, and home care services
Handle complex cases including locating family/support systems
Work on guardianships, capacity evaluations, and certificates of competency
Collaborate with legal teams and attend court hearings with patients
Address social, medical, and placement-related needs

Requirements:

RN with Inpatient Case Management experience (SW preferred)
Strong experience in discharge planning and care coordination
Ability to manage complex psychosocial and legal cases

Patient Focus:

Inpatient population requiring medical, social, and placement support services

Additional Details:

Holidays: Required as scheduled
