Registered Nurse – Case Manager

Note: 3 Years Experience Required • Inpatient/Acute Care Background Required • Floating between Bel Air & Aberdeen (10-mile radius) • Business Casual Attire • Weekends Not Required (Optional)

Position Overview:
The Registered Nurse Case Manager is responsible for coordinating patient care, discharge planning, and resource management in an acute care setting. The RN ensures safe, timely, and cost-effective care transitions while addressing complex medical and social needs.

Job duties:

Perform comprehensive case management assessments and develop individualized care plans
Coordinate discharge planning including placement, home care, and post-acute services
Manage complex cases involving guardianship, social situations, and long-term care planning
Collaborate with interdisciplinary teams to ensure continuity of care
Facilitate referrals to outpatient services, rehabilitation, and community resources
Conduct SDOH (Social Determinants of Health) screenings and address patient barriers
Ensure compliance with CMS guidelines, insurance requirements, and regulatory standards
Assist with insurance authorizations, benefits verification, and appeals/denials
Coordinate durable medical equipment (DME) and follow-up care services
Maintain accurate documentation in EPIC and ensure timely communication across care teams

Requirements:

Licensure: Active Maryland RN License or Compact License

Experience:
Minimum 3 years of inpatient case management or acute care experience
First-time travelers accepted with strong inpatient experience

Certifications:
BLS (Preferred)

Other requirement:
EPIC experience required
Strong knowledge of discharge planning, care coordination, and regulatory guidelines
Ability to manage complex medical and social cases
