Job Description:

Seeking an experienced Home Health RN for a 13-week assignment in Leander, TX. The RN will provide skilled nursing care to patients in home settings, complete comprehensive assessments, coordinate interdisciplinary care, and develop individualized care plans. Candidate must be comfortable working independently in the field and managing same-day patient visits.

Requirements:

• Minimum 1 year of recent Home Health experience required.
• HCHB (Homecare Homebase) and Oasis experience required.
• Active RN License and BLS certification required.
• Must be willing to participate in on-call rotation and weekend coverage as needed.
• Must be willing to travel up to 50 miles from home base.
• First-time travelers accepted.
• Local candidates accepted within 50-mile radius.

Responsibilities:

• Provide skilled nursing care and patient assessments in home settings.
• Complete HCHB and Oasis documentation during patient visits.
• Manage approximately 4–6 patient visits per day.
• Coordinate care with physicians, patients, families, and interdisciplinary teams.
• Educate patients and caregivers on disease management and treatment plans.
• Develop and oversee individualized plans of care.
