Description
Job Title: MSW – Case Management
Location: Everett, WA
Shift: 5x8 Days | Monday–Friday | 7:30 AM – 4:00 PM
Duration: 13 Weeks
Job Description:
Seeking an MSW (Master of Social Work) for a Case Management role in a Level II Trauma Center. This position will primarily support the Case Management Department with insurance authorizations, post-acute care referrals, Medicaid notices, discharge planning, and care coordination.
Candidates should have at least 1 year of experience and strong Epic charting experience. Acute care or hospital-based case management experience is preferred, but first-time travelers are accepted.
Responsibilities:
- Assist with prior authorizations and insurance reviews
- Coordinate post-acute referrals and discharge planning
- Deliver Medicaid notices and support patient care coordination
- Communicate with internal and external healthcare teams
- Document patient information in Epic
- Support discharge needs including DME and home care services
Requirements:
- MSW Certification/License
- Minimum 1 year experience
- Epic experience required
- Strong communication and coordination skills
- Acute care background preferred
