Description
Location: Mansfield, OH 44904
Duration: 13 Weeks
Shift: Days, 8:00AM – 5:00PM (M-F) | HPW: 40
Schedule: Rotating weekends (approximately every 8th weekend)
Requirements:
- Minimum 1 year of recent Home Health RN experience required.
- OASIS experience is REQUIRED.
- HCHB (Homecare Homebase) EMR experience is REQUIRED.
- RN License
- AHA BLS Certification required.
- First-time travelers will NOT be considered.
- Average 6–7 patient visits per day.
- Patient Population: Geriatric.
- Territory covers up to a 60-mile radius from the assigned hub/office.
- Guaranteed 40 hours per week.
- No on-call requirement.
- Mileage reimbursement provided at the current IRS standard rate.
- iPad provided for documentation.
- Cell phone is NOT provided.
Additional Information:
- Timely communication and documentation are required throughout the assignment.
- Dress Code: Scrubs (any color); No jeans.

About
Nestled in the foothills of the Allegheny Plateau in north-central Ohio, just an hour's drive from Columbus, Mansfield has lots to offer, including a connection to Hollywood. The city served as the primary filming location for 1994's Oscar-winning Shawshank Redemption, and the Shawshank Trail brings visitors to many filming sites like the Historic Ohio State Reformatory.
