📌 Facility Location: 750 E Adams St, Syracuse, NY 13210
⏰ Shifts: Day 5x8-Hour (7:00 AM – 3:00 PM, Monday–Friday)
📆 Duration: 13 Weeks

Job Description:

• Provide comprehensive Case Management, Utilization Review, and Clinical Documentation Improvement (CDI) services in an acute care hospital setting
• Work Monday through Friday, 7:00 AM – 3:00 PM (Monday start required)
• Coordinate patient care, discharge planning, and resource utilization to promote quality outcomes and appropriate length of stay
• Perform utilization review activities and collaborate with physicians, nursing staff, and payers to ensure compliance with regulatory and reimbursement requirements
• Support clinical documentation improvement initiatives to enhance the accuracy and completeness of the medical record
• Acute care hospital Case Management experience is required
• EPIC EMR experience is required
• Minimum 1.5 years of recent acute care experience (within the past 2 years) in a hospital of similar size and level required
• Travelers must have experience in hospitals of equal or greater size or trauma level
• Active New York State RN license required
• BLS certification required
• Strong communication, care coordination, documentation, critical thinking, and interdisciplinary collaboration skills required
• Candidates must have an unrestricted professional license with no history of suspension, investigation, or professional liability claims
• Must successfully complete pre-employment health screening, drug screening, and background check
• Candidates should be fully committed to the assignment before submission, as requested by the facility
• Deliver patient-centered case management services while maintaining accurate documentation and adhering to hospital policies and regulatory standards
