Description

Facility Name: CARE MUST
Facility Address: 4075 EVERGREEN VILLAGE SQAURE, SUITE 230, SAN JOSE, CA-95135
Number of positions: SALINAS Territory

Start Date: ASAP , Compliance takes about 2 weeks
Shift: 8hr Days
Ratios: 1:1 (*May visit up to 6 patients per day; 30 productivity points standard)
Guaranteed Hours - 36 hours weekly But traveler must see 5-6 patients a day.
First-Timers Accepted: NO
Weekends: If Needed
Is On-Call REQ?: No
Locals Accepted: Yes


Experience Requirements:
OASIS Experience Required
1 YR of Home Health Experience
Case Load: Home Health
Certs REQ: BLS
License: Must have CA license

Additional Notes:
Traveler is expected to have their own transportation.
System does a good job grouping clients minimizing drive and windshield time.
Occasional maximum radius of 50 miles.
Timely communication and documentation required
Documentation tools will be provided, and must be returned.
