Description
Territory: Walnut Creek, Lafayette, Orinda, Moraga, San Ramon (up to a 60-mile radius)
Required Experience
• Minimum 1 year of recent Home Health RN experience
• OASIS experience required
• HCHB (Homecare Homebase) experience required
• Active RN license required at time of submission
Patient Volume
• 30 patient points per week
• Approximately 5-6 patient points per day
Schedule
• 40 guaranteed hours (productivity requirements must be met)
• Holidays and approved RTO excluded from guaranteed hours
• Two weekend days per month
• No on-call requirement
Patient Population
• Primarily Geriatric
Additional Information
• First-time travelers accepted
• BLS certification required
• Travelers should be flexible with assigned territory
• Local candidates accepted at the travel rate
• Scrubs (any color) required; no jeans
• iPad provided
• Cell phone not provided
• Mileage reimbursed at the current IRS standard mileage rate
