Job Summary

Cuready Healthcare Staffing is seeking a qualified RN Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.

Responsibilities

• Coordinate patient care planning and discharge processes across hospital units.
• Utilize care coordination and discharge planning skills to optimize patient outcomes.
• Collaborate with interdisciplinary teams to assess and implement patient care plans.
• Provide case management within diverse hospital settings including Med Surg, LLOS, and ED.
• Float between units including Colby and Pacific campuses to deliver consistent care.
• Maintain up-to-date documentation and chart accurately using EPIC systems.

Qualifications

• Valid RN Licensure and BLS certification required.
• Minimum of 1 year acute care experience, including discharge planning and care coordination.
• Strong preference for candidates with diverse case management experience.
• EPIC charting system experience necessary.
• Willingness to work every other weekend as needed by the facility.
• Open to floating across hospital units and willing to work at multiple campuses.
