Job Summary
Cuready Healthcare Staffing is seeking a qualified RN, Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.
Responsibilities
- Coordinate care for patients with heart transplants, focusing on immunosuppression and infection management.
- Monitor post-heart transplant patients for signs of organ rejection and general health status.
- Communicate effectively with multidisciplinary team members, patients, and families regarding patient care issues.
- Assist in assessing post-heart transplant graft function, determining needs for additional assessments or interventions.
- Facilitate patient activities with consulting departments and physicians to ensure comprehensive care.
- Assess and address educational needs of patients and families, coordinating with appropriate services.
Qualifications
- Current CA RN License.
- Basic Life Support (BLS) certification from the American Heart Association.
- 1 year of experience in ambulatory care.
- Experience in cardiac nursing, specifically with heart failure and/or heart transplant recipients.
- Ability to float to other UCSF facilities as needed.
- Familiarity with Epic charting system and immunosuppressive therapies.
