Job Summary
Cuready Healthcare Staffing is seeking a qualified Registered Nurse - Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.
Responsibilities
- Coordinate patient care and facilitate effective treatment plans
- Conduct thorough assessments of patient needs and resources
- Collaborate with interdisciplinary teams for comprehensive care
- Implement discharge planning and care transitions
- Chart patient information accurately using Epic
- Participate in family meetings and utilize evidence-based guidelines for case management
Qualifications
- Current Registered Nurse (RN) license in the state of California
- Minimum of 2 years of experience in case management
- Experience with Epic charting system
- Ability to work independently and manage time effectively
- Strong understanding of CMS guidelines and insurance processes
- Travel experience preferred
