Job Summary

Cuready Healthcare Staffing is seeking a qualified RN, Outpatient Clinic to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.

Responsibilities

Provide chronic disease case management for adult day health program participants.
Monitor medical conditions in conjunction with the interdisciplinary team and primary care providers.
Educate and support caregivers to prevent nursing home placements and reduce hospitalizations.
Utilize Turbo TAR EMR system specific to California Adult Day Health Care industry.
Adjust and adhere to established patient condition parameters provided by primary care providers.
Manage a daily patient ratio of 1:40.

Qualifications

Current Registered Nurse (RN) license in California.
Minimum of 1 year RN experience; clinic, primary care, or home health experience preferred.
Basic Life Support (BLS) certification required.
Experienced in using EMR systems; prior experience with Turbo TAR is an advantage.
Strong communication skills and ability to work collaboratively with interdisciplinary teams.
Must have reliable transportation and be willing to travel locally as needed.
