Job Summary

Cuready Healthcare Staffing is seeking a qualified RN, Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.

Responsibilities

• Coordinate patient-centered care across interdisciplinary teams to ensure optimal outcomes.
• Manage patient discharge processes, ensuring adequate post-care instructions and resources.
• Facilitate communication between care teams, patients, and families regarding treatment plans.
• Utilize Epic for maintaining accurate and up-to-date patient records.
• Perform regular needs assessments and adjust care plans accordingly.
• Work every other weekend as required, adhering to the facility's staffing policies.

Qualifications

• Current RN license with active, unrestricted status.
• Minimum of 2 years of acute care experience in a hospital setting.
• Experience with case management and care coordination.
• Proficient in Epic or similar electronic health records systems.
• First-time travelers are accepted with clinical approval.
• Flexibility to float between departments as needed based on staffing requirements.
