Job Summary

Cuready Healthcare Staffing is seeking a qualified Registered Nurse - Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.

Responsibilities

• Manage a daily caseload of 15-20 patients, providing comprehensive care coordination
• Complete initial assessments, discharge planning, and utilization review
• Collaborate with interdisciplinary teams for effective patient care management
• Utilize Interqual criteria for case management and discharge processes
• Facilitate communication and coordination among all parties involved in patient care
• Adjust priorities throughout the day to meet patient and facility needs

Qualifications

• Current RN license and BLS certification required
• BSN required with experience in case management within an acute care setting
• Proficiency in Cerner EMR and familiar with Interqual criteria preferred
• Prior experience with California Dignity facilities is essential
• Outstanding collaboration and communication skills
• Ability to handle various payers, uninsured, and homeless populations effectively
