Job Description:

Supplemental Health Care is hiring compassionate and experienced Hospice RNs to provide patient-centered care for individuals and families in San Francisco, California, and surrounding areas.


Qualifications:

  • Active, unencumbered RN license in the state of assignment
  • Current BLS certification (American Heart Association)
  • CHPN certification preferred, not required
  • 1+ year of recent experience (within last 3 years) in hospice, home health, palliative care, or acute care
  • Strong knowledge of symptom management, pain control, and hospice documentation (CMS CoPs, Medicare Hospice Benefit)
  • Clear background and driving record; current TB test and immunizations
  • Reliable transportation and valid auto insurance
  • Flexibility for travel, occasional evenings, weekends, and on-call shifts

Hospice RN Contract Details:

  • $1,822 - $2,002 per week*
  • Nurse will have a designated territory of coverage
  • 4- 6 patient visits per day (45 - 90 minutes each, depending on need)
  • Travel time included in scheduling
  • All nurses on contract with SHC are paid weekly

Key Responsibilities:

  • Perform comprehensive assessments and develop individualized hospice care plans
  • Deliver skilled nursing care, symptom management, and emotional support
  • Educate patients and families on hospice philosophy and comfort care practices
  • Collaborate with the interdisciplinary team to review and adjust care plans
  • Maintain accurate documentation in compliance with hospice standards
