Professional Credentials
- MGB Badge Photo: Recent color photo (within 6 months), plain background, no selfies or filters, no eyeglasses, symmetrical headshot.
- License/Certificate Verification: Primary source verification of nursing license required within 30 days of start. MA Conditional Licenses not accepted. Both NURSYS and BON verification required.

- MGB References: Two recent references, at least one supervisor or above in skillset.
- MGB Skills Checklist: Required for submission.
- MGB Confirmation of Assignment/POI Form: Required for all initial assignments and extensions.

- MGB Criminal Background Check: Within 45 days prior to start, including international searches if applicable.
- MGB Massachusetts CORI: Criminal history for MA going back to age 17, within 30 days prior to start.

- MGB Exclusion Checks: Search OIG, GSA, MassHealth & NH Medicaid exclusion lists; attestation required.
Health and Immunization Requirements
- Fit Test: N95 fit test within 365 days of start date, updated annually. No religious exemptions.
- Respirator Medical Clearance (RMC) Attestation: Required if fit test is required.
- MMR Vaccine or Immunity: Documentation of two measles, two mumps, and one rubella vaccine or two MMR vaccines, or proof of immunity via blood test.
- Hepatitis B: Strongly preferred to have 3 vaccines or proof of immunity or declination.
- Influenza Vaccine: Required during flu season (Sept 15 - April 30). No exemptions.
- TB Test: Documentation of TB skin test or negative QuantiFERON/T-spot blood test within 3 months of start, updated if >6 month break.
- Positive PPD - Negative Chest X-Ray: If positive TB test, chest x-ray documentation required.
- Health Statement: Physician statement signed by MD/DO/NP/PA within 365 days of start.
- Tdap Vaccine: Preferred within 10 years and updated upon expiration.
- MGB TB Symptom Survey: Required if positive PPD, updated annually.
